CARE SKILLS: Assignment 2

History
Mrs.Margaret is a 90 years old lady who is on full assistance in all the activities of daily living. She suffers from osteoporosis and had broken a femur bone from a fall at home. A bone density scan shows that she has paper thin bones. She was brought by her daughter to XYZ nursing home as they were not able to look her after at home. Margaret is confined to bed and to her ‘rise and reclining’ chair which is comfortable.

Assessment of Risk Factors and pain

It is important to maintain the skin integrity of Margaret as she is more likely to have pressure sore. Special care has to be given while turning the sides, lifting her and changing her clothes as she may break her bones. She is suffering from great pain. Though she doesn’t want others to know about her pain, often her facial expressions and moans make it clear that she is suffering from pain.

Individual special needs

  1. PHYSICAL
      • Has to be given bed bath
      • Washing and feeding are required. She is provided with porridge, tea with thickeners and food that are easy to swallow as doesn’t have dentures.
      • Has to be turned the sides in every two hours so as not to get pressure sore.
      • Moving her from the bed to her chair and back to her bed is done by two carers lifting her manually as she is afraid of hoists.

  2. EMOTIONAL
      • She doesn’t express her feelings
      • The pain while moving her upsets her
      • She does not like hoists
      • The illness and dependence has emotionally upset her.

  3. SOCIAL
      • She is taken to the activities room as she likes to sit there along with the other residents and listen to music, watch them dancing and doing some exercises.
      • She wants to have her food in the dining room as it gives an opportunity to see the other residents.
      • She tries to speak to other residents while in the activities room and in the dining room but since it is not audible they don’t...
