In accordance with paragraph 34 of the Immigration Rules, this form is a specified form for the purpose of the Immigration Rules as of 13 December 2012 and must be used for all applications made on or after that date for the purposes stated on this page and listed in section 2. You should complete this form only if you wish to apply for leave to remain in one of the categories listed on the right hand side of this form. If you are not applying for leave to remain under one of the categories described on the right hand side you are completing the wrong form.
