1. Understand person centered approaches for care and support.
1.1. Person Centered Care is an approach to care that respects and values the uniqueness of the individual, and seeks to maintain, even restore their individuality. We do this by creating an environment that promotes personal worth and uniqueness, social confidence, respect, truthfulness, independence, engagement and hope.
1.2. Explain why it is important to work in a way that embeds person centered values
The care you carry out on a day to day basis should always be centered around the service user you are caring for. You are not there just to meet their physical needs but to maintain their sense of being a person and their identity. This involves you establishing a supportive relationship with the person, involving respect and trust. You should be listening to the service user, despite any difficulties, understand their emotional needs and preferences and learn about their history. By doing this you help maintain the service users identity, independence and self-esteem.
2. Understand how to implement a person centered approach in an adult care setting
2.1. Describe how to find the history, preferences, wishes and needs of an individual
The history, preferences and needs of an individual can all be found in a service user care plan. Each care plan comes with a person centered section showing how to work with that service user.
2.2. Describe how to take into account the history, preference’s, wishes and needs of an individual when planning care and support
After reading care plans, working with the service user and getting to know them I would cater the needs and wishes of an individual that way when creating activity charts, meal charts etc.
This would be done for each service user to guarantee that everything has been personalized for that one person and not copied over from someone else.
