The purpose of the following assignment is to discuss a patient case from a previous clinical placement, exploring the nursing assessment and the delivery of care which the patient received. The individual focused on within this assignment will be kept strictly anonymous, referred to as “Jean” adhering to the NMC ‘The Code: Standards of Conduct, Performance and Ethics for Nurses and Midwives’ (NMC, 2008); ensuring that all patient’s information is maintained strictly confidential; this being a crucial element within any professional setting (NMC, 2008). Jean was admitted to hospital due to exacerbations of her COPD, this highlighting increased breathlessness and Jean displaying difficulty in communicating; only able to speak in short sentences. There was also identification of increased sputum purulence and an increased wheeze. Jean has a past medical history of type 2 respiratory failure and previous PE (Pulmonary Embolism). Jean lives alone, with support from carers twice daily. She has the ability to mobilise with the use of a walking aid; however this was only for very short distances as her exercise tolerance has reduced, requiring a wheelchair for longer distances. Throughout Jean’s duration in hospital, she was seen and reviewed by various healthcare professionals including the COPD specialist nurse, who confirmed Jean’s condition had deteriorated and had now reached end stage COPD. Jean appeared to be very low in mood, lonely, anxious, extremely sleepy and breathless the majority of the time. The nursing assessments that are being examined will be in relation to breathlessness; this being an actual problem experienced by Jean.
