What is Reflective Practice?
Reflective practice is a process by which you: stop and think about your practice, consciously analyse your decision making and draw on theory and relate it to what you do in practice.
Critical analysis and evaluation refocuses your thinking on your existing knowledge and helps generate new knowledge and ideas. As a result, you may modify your actions, behaviour, treatments and learning needs.

Reflecting upon your daily work is vitally important in a health and social care setting. Every day is always different from the other and can be unpredictable most of the time. It is vitally important to be observant, assessing the individual on each visit making notes of any changes that are taking place in their health and environment or surroundings. Report any findings and concern to the nurse on duty, OT or physio therapist so that they are aware of the situation. This report can be done over the phone and also should be documented in patient’s folder making sure a copy is to be brought back to the office and placed in patient’s folder. The folders are brought each morning for qualified staff to receive hand over through documents from recent visit to review reflect and on.  

In an incident that I came across during a late shift, I noticed that Mrs C was struggling with her walking.
