1.1 Describe the duties and responsibilities of own work role
My role as a senior outreach worker within the referral and assessment team RAS is a very diverse and changing job role. My core duties are to Support the nurse led service to manage clients/patients who are currently in a Crisis situation. I support the nurses at times while they conduct Needs and risk assessments with a person that has been referred to RAS via the authorised pathway. I am also required to Visit patients in the community to support them during the crisis period. There are many different ways to offer support and this is always highlighted within the patents Needs/Risk assessment and in the care plan. To do my Role well I need
• Ability to listen effectively
• Have excellent communication skills with service users/colleagues and other professionals
• To work unsupervised in a range of settings, office, and community visits etc.
• To provide practical support with social inclusion/engagement/benefits
• To work in accordance with the Recovery Model/WRAP
• To adhere to the care plan and provide crisis support as identified by the keyworker/nursing team
• Additional duties: Assist team with up to date files that comply with audit tool. To keep the office whiteboard up to date, transferring visits to the allocation diary.
• To attend reflective practice and psychological training
• To keep up to date with all mandatory training
• Work within the guidelines of set codes of practice.
1.2 Explain expectations about own work role as expressed in relevant standards
Within my role as outreach worker which would be classed as support worker I would have to operate within the scope of my job description but I would also have to operate within the National Minimum Standards NMS for healthcare support workers.
