Identifying appropriate information to enable individuals to make informed lifestyle choices is an extremely important part of my day to day job. It is also vital for the patient to be given the right information. It would be awful if a patient was given the wrong information as it would lead to poor decision making. It is not my job to give patient’s and individuals false hope as it can break trust in your professional relationship. In regards to the individual’s lifestyle choices I do this in my job role by working closely with the patient who has suffered the stroke on my ward to assess the individual’s need for home and their life styles. It all begins by assessing the patient to see how much the stroke has affect them. However through out the stages of their rehab their situation may change quite dramatically. For example the individual may be a hoist transfer at the beginning of their rehab and may get to the stage of walking with a frame at the end. These are just a few of the things that can have a huge effect on the information given to the patient to make a decision on their life style choices. If the patient cannot progress without being a hoist transfer it maybe the individual need to go to a nursing home for 24 hour care. If the individual can manage to sit out via step round transfers only it could be we have to give information about going home with a community team to help them at home for a few weeks post their discharge date.
If the patient cannot mange walking up the stairs we may talk to them and their families in regards of the idea of downstairs living if there is a toilet down stairs. This would impact the individual greatly. These are a few examples of identifying appropriate information to enable individuals to make informed lifestyle choices.
